Unleashed My Dog Training Class Service Agreement

VACCINATIONS: Owner agrees their puppy is current on rabies, DHLPP and Bordetella vaccines (per their veterinarian’s recommended vaccination schedule considering age of puppy).

SAFETY: Trainer can dismiss from class any puppy they feel is aggressive, unhealthy or out of control. In this case no refund is given.

CLEAN UP: Owner must clean up after their puppy—inside and outside. (Supplies provided).

LIABILITY: Owner agrees to indemnify and hold Trainer harmless from all liability or any loss, damage or injury to persons, animals or property arising from or related to owner’s puppy’s behavior before, during, or after class. Trainer shall not be liable for loss or damage to owner’s puppy for any reason unless said loss or damage was a direct result of Trainer’s negligence. Owner will assume full financial/moral responsibility for the action of their puppy.

Bring payment of $125 to first night of class. Check or cash only. Make checks payable to Unleashed My Dog LLC.
